Simply create a login account through the link below and provide the requested information. You will have 90 days to complete the process from beginning to submission. If your application has not been completed within that time period, it will automatically be placed in "exit" status. 

There is no deadline to apply to become an OK-LSAMP Scholar. However, the sooner you become a part of the program, the more benefits you can receive.
